Agreements
To provide the best service possible, situations may arise that would require IT’S A BLAST to share information with the school or vice versa. I consent to information sharing with my child’s school.
I understand that if I wish to cancel services with It’s A BLAST Program I am responsible to notify It’s A BLAST Program in writing, by letter or email, on or before the 15th day of the PRECEDING month in which I wish to cancel services, or I will be charged the full fee for the month (i.e. cancellation needs to be on or before August 15th to cancel for September).
Subsidy Deposit - It's a BLAST requires a subsidy equal the maximum amount of subsidy $366. If the program you are registering for is less than $366 the deposit will be equal to the program cost. Payment arrangements are available.
I understand that I am responsible to have my childcare fees paid in advance, by pre-authorized credit card or debit agreement. If any payments are returned NSF, I understand the NSF payment plus a $30.00 service charge must be replaced by cash, debit, or credit card within 5 business days and failure to do so may result in the termination of all services.
If a child does not arrive on a day they are scheduled, and the BLAST office has not been notified by a parent, we may have to make several phone calls to locate the child. This takes a staff member out of required ratios and distracts them from the supervision of all children in our care for a period of time. You will be charged a $25.00 Child Finder Fee if calls are required. This fee is per family, per occurrence. The program staff will give two verbal warnings of this policy and then on the third offence fees will begin.
I understand that I am responsible to pay any Late Pick-up fees if my child is picked up from program after 6:00pm. A charge of $25.00 will be incurred for any pick-ups between 6:00pm and 6:15pm, with an additional charge of $25.00 for every 5 minutes after that, until my child is picked up. All Late Pick-up fees must be paid within 5 business days.
Receipts are emailed monthly to the registering parent and can be used for tax purposes. If an official tax receipt is requested, a $25 administrative fee will apply.
I give permission for IT’S A BLAST staff to administer First Aid treatment, as required, according to their training. First aid treatment can be as simple as applying bandages to minor cuts to more serious cases needing emergency officials. In the case of an emergency, BLAST will make every effort to contact the parent. In the event that they are unable to do so, I give permission for medical procedures deemed necessary by medical professionals to proceed. I, hereby, indemnify the IT’S A BLAST Program for any costs in respect to medical services.
It’s A BLAST Program has a Social Media Policy which states; Parents and family members of BLAST children need to refrain from utilizing social media for the purpose of negative behavior. Any threats to BLAST, negative feedback regarding your child’s care, staff complaints, office staff complaints, or negative comments of BLAST as a whole agency will not be tolerated and could lead to termination of your child’s care.
Our program has a digital/video camera which is used to capture activities during day to day programming. I give permission for pictures of my child to be used either at the program, in a scrapbook, or on the It's A BLAST website (names WILL NOT be published with any pictures on the website)
Parents will be required to sign an activity specific waiver form before children participate in an offsite activity.
Profanity, confrontational behavior, and physical or emotional abuse will not be tolerated. If such behavior is displayed, you will be asked to leave and your services may be terminated.
Further, I hereby release the IT’S A BLAST Program and its employees for all liability for any personal or consequential injury or illness occurring at the IT’S A BLAST Program.
The IT’S A BLAST Program reserves the right to discontinue service for non-compliance of program conditions. On admission of my child / children to the Lethbridge Community Out of School Association – IT’S A BLAST Program, I agree to comply fully with the requirements and policies of the IT’S A BLAST Program and the terms of this agreement.
I have read and agree to the above terms and conditions. I understand that this agreement is valid as long as my child is registered with the IT’S A BLAST Program.
